Please fill out this form when requesting leave.
All time off submitted within 48hrs of the time off request will be refused unless authorised by your line manager.

This form does not ensure the time off will be granted.

We will aint to grant all leave on a first come first serve basis, however medical appoints will take prioirty (Proof may be require if this creates a conflict).
